I expected him to say something like "hi. How are you doing?" Instead he said he did not know who I was. What is going on? Could he have forgotten me that quickly? It was only about four months since I last saw him. Is he playing some kind of game? I was so stunned at his reaction that I did not know what to say. I simply hung up. Should I call him back or just forget about his strange reaction?

To be honest, i have had text messages and thought who is that, as i haven't programmed them into the phone.
He seemed to be curious as to who wanted to say hi how are you. In your thread, i am assuming you left your name at the end...
Your name could be common? And, 4 months ago is a long time.
Not sure if you dated him but again, if it was causal he may have dated a few since over 4 months, again if you did, you don't say how long for? If you didn't and you were ok at work, he could be thinking the message was from someone he dated?
A simple misunderstanding i think...
Perhaps you need to explain a little more so we can answer what you maybe should consider from here...
