I've used them but as a backup to another method. Personally I didn't want to use them as my sole method of birth control. I think the effectiveness is too low and the odds of it not being used properly are too high. If you go to a doctor you can get a cervical cap or diaphragm fitted and then you put the spermicide in there. It provides some barrier protection and keeps the spermicide in the correct place. I found I can't tolerate spermicide so I never went that route and did hormonal based birth control instead.
