Sounds like a blood blister. They are usually causes by pinching, in this case, not the whole nipple. I suppose something like a needle prick or wire prick could do the same thing if the hole closes completely on the outside but not under the skin.
ooook... rather embaressed to have to share this with you all, but duty calls and Id like some advice if at all possible.
Ever since i can remember i have never had 'smooth' nipples shall we say - they have always appeared cracked. (just a bit of 'too much info' for you there!)
A few days ago, i felt a strange twinging pain - not agony pain, but more discomfort and sensitivity in my right nipple. I ignored it, but everytime i moved or when in a bra, i could feel this discomfort. I went to bed last night making sure that i took off my bra, thinking that my nipple may have been rubbing against my bra or something like that, but this morning the discomfort came back, and is still there.
I took a closer look at the nipple, and it would appear that there is a tiny redish black dot right in the centre of the nipple, and when i brush my finger over it, it moves. its like a tiny bud in the centre of my nipple.
Im not sure whether to buy a certain cream to see if that helps - but I have no idea which ones to buy.
I dont suppose anyone can shed some light on this, or if anyone has expereinced this before?

Its strange because it doesnt look like a blood blister, although im thinking of the blood blisters that you can get on your hands, so maybe blood blisters can look different on various parts of your body? - i really dont know.
Its definitly attached to the middle of my nipple thats for sure. like a piece of 'nipple bud' (ha!) has come away. The sensation of it is really starting to annoy me. Im not sure whether a trip to the doctors may be needed? or is that over reacting?

If it is a blood blister, you could lance it with a sterile needle. It will be tricky because of all of the nerve endings increase the pain. Blood blisters sometimes produce pronounced bulges.
ooo - id probably pass out if i put a needle anywere near my nipple. ha! and yeah you are right, if its causing a weird kind of pain at the moment, god knows what it will be like if i lance it wil a needle. I dont have particularly steady hands, and i just know that i will be shaking like a cacking dog incase i pierce the whole nipple. LOL!
Do you know of any creams that may help? thats more on the lines of what i was thinking, but i dont want to end up irritating it even more.

I don't know of creams that would help, but possibly sun burn lotion would desensitize it. I'm not sure what precautions, if any would be needed at the center of the nipple.
