Question pelvic pain - when to consider ER

I have an ovarian cyst, tipped uterus, and fibroid tumors outside the uterus. I recently had to stop taking NSAIDS for pain due to problems from their use. Now my pain is very intense. Not constipated nor diarrhea. absolutely no chance of pregnancy. pelvic area feels tight inside and feel something putting pressure whenever I urinate or have a bowel movement. Any other options outside narcotic pain medication when regular medication doesn't even touch the pain. Narcotics do relieve the pain but I prefer not to have to use them constantly for the constant pain. Help!
