blood when i am not on my period

this has been happening for a while now.
i have my period which is totally fine. but say about a week later i notice i am bleeding again, not much, just when use loo paper i notice it.
but this time i've had to use a small sanitary towel and the blood is coming out in like 'clumps'.

hope its nothing to worry about =/
and i'm 17 and started my periods about 2 years ago and this has been happening for about 2/3 months.
