Default Did I have a miscarriage?

To start out with, I am on no forms of birth control. The only birth control my husband and I use are condoms. I started to feel "different" around November/December, but I didn't take a pregnancy test because I had my period. I had a pap after that period, around mid-December, and it came back normal. Now, I have had 3 heavy periods in 5 weeks. All full periods, all abnormal. My lower back hurts quite a bit most of the time. I feel fatigued and weaker then usual. I have also noticed on my period before this one that there were "chunks" in the toilet. I also have/had a clear, membrane looking discharge along with the blood. I have not taken a pregnancy test yet, should I? Can it come back positive even if I miscarried? If I did miscarry, I would only have been a few months along, is it possible my body pushed the fetus out? Also, would my doctor had noticed I was pregnant just by having my regular pap? I would have only been a few weeks along at that point. Thank you in advance for your replies.
