This is one of those things you just do and its hard to say how. Like trying to explain the finer points of using a clutch after you've been driving one for years.
You can just let some of it dribble out of the side of your mouth.
You don't have have him all the way into your mouth, let your hand(s) take part of his penis and you won't feel so much like you are going to choke or drown.
