when i get my period the first day i have such severe cramps that it literally causes me to scream in pain. i'm not being over dramatic it feels as if someone is performing surgery on my uterus while i'm awake. i dont even know what to with myself when it happens. i've tried medicines and just "relaxing" but its so painful that most of the time i end up rolling around in my bed screaming and crying with the pain. i'm 21 years old and this has only recently (about the past 6 months) starting happening. i'm at the point where i dont know what to do. if anyone has any thoughts or has experianced this sort of pain i would appreciate it.

Unfortunately many women go through this. I personally am one of those women. Cramps have been compared to being nearly the same as the contractions a woman would have while giving birth. We all know how painful that can be just by seeing a woman give birth.
There are some things you can do to help with this. Make sure you keep a record of when your period should start on your calendar. Also make a note of when your cramps usually start and end. My cramps are usually the worst on the first two days. From now on, anticipate when your cramps are coming and prepare for them. You can do this by avoiding caffiene and aspirin for a few days before your period and then during your period. Both things effect the blood flow and can increase cramping. The day or the night before you expect your cramping to show up, start taking pain killers such as Ibuprofine. I take 2 Advils just before I expect my cramping to start. It takes some time for them to kick in so it's best to be on top of that. Then, if you start to feel some pain, take one more Advil. You can take up to six in 24 hours. Of course, you want to take as little as possible. Make sure you eat before taking them too.
Since you have such bad cramps, you should avoid using tampons unless you absolutely have to. Tampons can cause further irritation in the vagina and put unwanted excess pressure on the uterine walls. Also, once a tampon fills with blood, the blood starts to fill in your vagina. The more blood that collects in there, the more you will cramp. The cramping seems to be a result of your body telling you to push the blood out. I always know if a tampon is full because my cramps start to get bad. Using a pad will reduce the cramping and also allow the uterine tissue and blood to discharge much faster.
Hot compresses, showers, or baths will help with the cramping. I always end up in the bottom of the bath tub with the hot shower beating on my lower back. It seems to help with pain and also to calm me.
Some women will say that staying active helps them. I personally have more pain when I am up walking around, but you might want to experiment to see what is right for you.
Hope this helps!
