If you guys have hada rough year than you both might need a time out to relax and be with each other?

Maybe a night away? Somewhere with a relaxing environment e.g. a spa, candles etc.

A friend of mine does a commitment ceremony every year for their wedding anniversary and gives a whole speech about his wife etc. They bascially renew their vows every year with family and friends.

I think some quality time together with nothing from your busy everyday life would be nice. Just to appreciate one another and share stories about the good times.

My husband is quite big on doing things like that and I have to be honest I remember that more than the material things.

For our upcoming first wedding anniversary, I am thinking of putting a slideshow together of all the photos throughout our years together with background music being our first song as husband and wife.

    I hope that helps you.

Think about what you most miss in your relationship and do that.

If it is new experiences - do something new: go somewhere you havent gone before , vist the tourist sites in a nearby city - even if they are minor. Eat at resteraunts you haven't tried before.

If it is intimacy, then spend the day in bed. Try something you haven't done before.

If you are overworked, go to a local park and just walk around, sit on the benches, feed the ducks.

If it is excitement, try something new - skiing, bunjee jumping.

If it is silliness, go to your local mini-golf place. or go to the amusement park and ride the rollercoasters.

Do all of the above - though not at the same time <g>

Most importantly, don't try to make it a SIGNIFICANT EVENT (echo...echo..echo), just have fun to remind yourselves of why you love each other.

I think what you should do depends entirely on who you are. I think material gifts are not a good idea - spend the money on doing something fun.
