So basically how do you bring up the subject with someone that you are dating and what questions would you feel necessary to know?

Whilst I do not, and have not ever felt the inclination (nor the right) to pry into someone else's sexual past I do not think it is unreasonable to expect someone to be sure that they are sexually healthy, that is to say STD free. If they are not sure then you could suggest you both go to clinic before sleeping together - or use condoms - before you start having unprotected sex.
Based on your post, I get the feeling that you are (no offense) relatively inexperienced and would like to establish exactly how experienced your partner is and possibly how many partners he has had? I think you may be torturing yourself a little if you insist on asking all of the details and he obliges by disclosing them to you, in all of their glory detail.
Before I extrapolate any further if you would like to share with a little more and perhaps state exactly what it is, or what kind kind of things you would like to know we can advise you a little more specifically. All you have to do is search some of the posts here and you'll likely find your situation is far, far from unusual.No matter how objective we try to be, we all see through our own eyes... -
Depends on what you want to talk about sex. Are you talking protection and contraception, fantasies, sexual needs and desires, etc. They come up during different stages of the relationship and during different times. For example, you might want to have the protection and contraceptive talk early on before your start having sex but after you know that is where you are heading. You could just bring it up as "I really like you and things are going really smoothly. I totally see this becoming sexual very soon. I just want to make sure we are clear on projection as such. I always use condoms. It covers both the STI aspect and the contraceptive aspect. Maybe for our next date we can go back to my place. Is there someone special in your life with whom you are considering being intimate or is this just for future reference?
How comfortable are you with your own body? have you ever explored it? masturbate much? etc.?
About the only question I would consider with someone I don't know, with whom I am considering being intimate, also assuming the other person is male, would be, "Do you have a condom?" (you may want to purchase some yourself just in case).
Protection is key. You protecting you from him. Even after you have dated someone for a while...protection is key...until they are ready to go and get tested with you for STDs.
Being open and honest is not only 100% necessary but it can be very sexy too.
For the record...facts and data don't lie...people do. So go and get tested together before you have unprotected sex with anyone. No test, no unprotected sex. It's that simple.
