When you have sex a lot of things happen that don't at a doctor's exam. First you will produce more lubricant. Also the perception of pain is lessened by the hormones released. There should also be stretching from at least fingers if not a sex toy prior to penetration. Using a sex toy a few times before you attempt sex with someone can make it completely painless. Another thing is that if you had a pap smear they take a little piece of the cervix which can cause various levels of pain that won't exist with sex. Some women have a very sensitive cervix.
