I am a guy who has suffered from premature ejaculation since I started having sex at 16. It was not until I was about 32yo that I saw a Doctor who specializes in this area. He put me on Zoloft tablets which are referred to as SSRI's and used to treat depression. I only take half a tablet a day which he tells me is quite a low dosage but I have gone from lasting 30secs to lasting 2-3 minutes of continuous thrusting. If I pause I can last a lot longer. For a guy who has suffered from PE for so many years this is a really good result.
For years I tried all the other methods that people suggest but unfortunately they did not work. I have not grown out of it and thinking of something else at the critical moment definitely does not work for me.
I suggest you get your boyfriend to talk with his Doctor and ask for a referral to a specialist in this area. The effort will be worthwhile as my sex life is now much more satisfying for both my wife and I.

Well I've never heard of smegma. That sounds weird!
As for the pre ejeculation, he's going to have to fix this himself. Kegals will most likely fix him IF he sticks to a strict regimen.
Another thing you could use in conjunction with this is a penis desensitizing cream - benzocaine, xylocaine, etc.Comment

Maybe he should try some breathing exercises. The one I know is where you inhale slowly, pause for the same time it took him to inhale and exhale at the same rate he inhaled. As for my understanding men ejaculate due to the build of tension and excitement. So by doing this breathing exercise would be a way to control that tension and excitement.Comment
