If someone is really tired they can fall asleep during sex - but we're talking truly exhausted.
I find it hard to imagine someone not waking up before they ejaculated - unless maybe if they were very drunk or stoned.
I don't think it was anything at all you did wrong (and you make me horribly jealous from your willingness to try so hard for him).
If he has gotten used to using his hand while you give him oral, maybe he has conditioned himself to only finish that way. (his loss)

Geez, we've both fallen asleep, doing and receiving all sorts of things. We've even woken up the next morning and realized neither of us knows who fell asleep first. We just laugh about it, but we've been together for 15 years, so I guess everything is bound to happen and reoccur at some point.Comment

I would say it's not possible, only when he has an involuntary wet dream. When my g/f stays I wake up with a huge erection but don't feel like sex. But I ask my g/f if she wants to use it before it goes to waste!! So she sits on top and gets her orgasm. But at that stage I can't ejaculate because my mind is not sufficiently stimulated to do so.
