I think the most important things is foreplay.

There is no where near enough foreplay to get a women to orgasm.

Because women take a lot longer to reach their climax they need a lot of foreplay.

When shes at the threshold of her orgasm then the guy can enter.

At orgasm her vagina begins to clamp his penis so this idea of girth may or may not actually be relevant ; still length could be a problem if hes too long and is bashing into the neck of the cervix.
