When I use penetration as part of masturbation I rarely use an in-out motion, I will put a dildo or vibrator in as deep as it will go and push to get the feeling of pressure deep in my vagina. That, along with clitoral stimulation, gives me a good orgasm.
Sex is very different. The feeling of bodies together, of someone else controlling the penetration, the fact that it's my lover's penis inside me, the different angles and intensity my man's penis and movement give me, kissing, looking in his eyes - none of those things happen in masturbation and really there is no comparison.
It is good that you are practising feeling something inside you but you won't know how the sex feels til you have it.
I don't recognise that tag of skin. I don't have one myself and the few women I have been sexual with haven't had one either. But that's not a very big sample of the female population.
I don't think that hurting will stop you feeling pleasure as well. I do think you need to find a different doctor who will take your concerns seriously.
As for the g-spot. I can't get to mine with my fingers at all. Nor can I stimulate it with a straight vibrator. My man puts two fingers in my vagina, (he's facing me and the palm of his hand facing him) and makes a beckoning motion with his fingers (you know, like calling a waiter over) - that hits it exactly for me. He also bought me a curved vibrator (not too curved, just like a banana) - when I use that I move it in and out of my vagina with the curve like a smile, not a frown, the top of it rubs my g-spot and it's a great sensation.
It could be that your surgery has left you too sensitive where you should be 'just right' sensitive. Again, I think you need to change doctors and say to the new doctor, very frankly, just what you have said to us.
