I am not self conscious of the size of my penis. Again, this comes down to whether the man has "proved" himself and therefore has confidence. If a man has had children, i.e. proved his fertility, he is a lot less likely to worry about his size. It has, after all, proved capable. If a woman were to criticise my size I would tell her at least it's bigger than hers!. No seriously, I don't know of any men who worry about this. Try a few days on a nudist beach and you worries will soon disappear!

I would never do anything about it, even if there was the technology. The penis is such a sensitive, and useful, organ, that it shouldn't be messed with.

The side-effects of breast enhancement I don't think would be as severe as with a penis (I know, I've heard horror stories, so I know some bad things can happen). If they mess up the penis, there's a big chance it might not be usable anymore. There's no way I'd take that chance.

That's the very reason why I would never get lasik done on my eyes. If they screw up, I could be blinded. Not taking that chance either.

Different things matter to different people. In this case though unfortunately the women most likely to want their partners to use an extender or large dildo are the ones with less endowed men. Those men are exactly the ones likely to be sensitive about their size and be upset.

I am about average in size and married 14 years.
